Effective system-based root cause analysis

Aim: 
This workshop will provide participants with a background on the importance of thorough root cause analysis. Through the use of case studies, and several standard root cause analysis tools, participants will be able to practice skills that can be transferred back to their practice or workplace

Description:
Healthcare is a complex system; understanding the root causes of poor quality or safety events requires a rigorous and intentional approach. Problems cannot be effectively addressed if the root causes are unclear. Taking system approach can lead to more sustainable and effective solutions, and avoids blame on individuals. Understanding standard root cause analysis tools such as fishbone diagrams and the ‘5 whys’, and having some practice using them, can enhance the participants knowledge and abilities to solve problems and create effective change.
